RELEASE OF LIABILTY

This Release is executed on ( date ) ______________________, by and between MESA KIDS COOKING SCHOOL LLC d/b/a  MKCS, an Indiana Limited Liability Company, and

  For and in consideration of my minor child being permitted to participate in the food demonstration, meal preparation and/or other activities at 216 Pearl St., New Albany, Indiana conducted by Mesa Kids Cooking School LLC d/b/a MKCS, its officers, members, employees, promoters, sponsors, advertisers, and lessees of the premises, Releasor does now, as the parent and next friend of my minor child, RELEASE, WAIVE and DISCHARGE Releasee and its officers, members, employees, promoters, sponsors, advertisers, and lessees of the premises, and each of them, from all liability to Releasor, as parent and next friend of my minor child, for any and all loss or damage, and any claim or damages resulting from the same, on account of injury to the person or property of Releasor’s minor child, even injury resulting in death of Releasor’s minor child, whether caused by the negligence of Releasee, its officers, members, employees, promoters, sponsors, advertisers, and lessees of the premises or otherwise while Releasor’s minor child is participating in the food demonstration, meal preparation and/or other activities at the premises. 

  Releasor, as parent and next friend of my minor child, agrees to indemnify Releasee, its officers, members, employees, promoters, sponsors, advertisers, and lessees of the premises and each of them from any loss, liability, damage or cost they may incur due to the presence of Releasor’s minor child in or upon the premises, whether caused by the negligence of Releasee, its officers, members, employees, promoters, sponsors, advertisers, and lessees of the premises or otherwise. 

 Releasor, as parent and next friend of my minor child, assumes full responsibility for the risk of bodily injury, death or property damage due to the negligence of Releasee, its officers, members, employees, promoters, sponsors, advertisers, and lessees of the premises or otherwise while my minor child identified above is in or upon the premises participating in the food demonstration, meal preparation and/or other activities at the premises. 

 Releasor, as parent and next friend of my minor child, agrees that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the State of Indiana and that if any portion of this agreement is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
