Please fill out the Medical Form completely . All allergies must be filled  in and reactions. We need this just in case of an emergency happens and we have the proper information. Please note we will not be resposible if there is any allergic reaction that was not listed. 

Release By Parent - A waiver  form  for kids to be able to participate in our classes. We take all precautions but accidents do happen and we want parents to be aware that we need  kids  to follow directions and follow rules in our kitchen for safety for all students to minimize any accident toward themselves or others 

The agreement is for both kids and parents. We ask that you read it with your child . Both parents and child need to  understand the procedures and rules. In order for everyones safety and to have a good enviorment in the classroom we need to have kids understand them before class.  

The release of photo and video  is for us to be able to share them on our FB page or promotional video, website and marketing.
